We will need the following when you contact us:

1. Your insurance information and your specialist co-pay amount.
2. Reason for your visit. Have you had any tests performed?
3. Name of referring doctor.

Please bring your picture ID, insurance card, and referral (if required by your plan) to your appointment.

SSA, PC participates in most insurance plans, all medicare plans, and some medicaid plans.

All financial arrangements will be discussed when a surgical procedure is scheduled. We accept cash, checks, and all major credit cards.
